The mission of InnovAge Greater California PACE (IGCAP) is to enable aging individuals to live with dignity in their own homes and communities. To enroll in our program, an individual must be 55 or older, live in our service area, be certified as nursing home-eligible by the State of California, and able to live safely at home and in the community, with the support our program provides.
Working with you or your loved one and family, the IGCAP care team works out a care plan and then provides the services needed for physical and emotional wellbeing. You and your care team set the frequency of visits to the Day/Health Center, and your care plan is monitored and updated as needed.
We help aging participants:
- live in the most independent setting possible; and
- enjoy the social and health benefits of interacting with other participants and professional caregivers.
What services are included?
InnovAge PACE can be an excellent option for individuals with limited financial resources or declining ability to perform activities of daily living. The program provides coordinated personal and skilled care in the home, as well as in the Day/Health Centers where socialization, meals and recreation are also available. A care plan is created for each program participant by an interdisciplinary team of professionals. Based on identified needs, the team works with you and your family to create a plan of care and outlines the services you will receive.
Our services include:
Medical care from on-site physicians, nurses and specialists
In-home care (chores and personal care)
Adult day program
Transportation to and from the center and medical appointments
Medical equipment and supplies
Physical, occupational, recreational and speech therapy
Dental/vision/hearing and foot care
Social Services, family support and counseling
Hospital and emergency care
Additional services not covered by Medicare and Medi-Cal, but determined necessary by the team

Who can Participate?
The individual must:
- Be certified by the State of California to need a level of care equal to nursing home services;
- Be at least 55 years old;
- Be able to live safely at home and in the community; and
- Live in the IGCAP service area.

*The only exception is if the individual lives in or moves to supportive housing (a nursing home), in which case there is a co-pay to that facility.
Please note: IGCAP participants must receive all needed healthcare services (other than emergency services), including primary care and specialist physician services, from IGCAP or from an entity, person or company authorized by IGCAP. IGCAP participants may be fully and personally liable for the costs of unauthorized or out-of-network services. Medi-Cal beneficiaries who have a share of cost remain responsible for their individual share of cost. Participants without Medi-Cal pay a monthly premium to cover long term care and a premium for Medicare Part D drugs. PACE organizations are regulated by the Centers for Medicare and Medicaid Services and the California Department of Health Care Services (DHCS). DHCS does not promote or endorse InnovAge or any provider over another. The cost of this website is fully covered by InnovAge.
