Senior care decisions are never easy, especially when a full range of complex issues needs to be addressed with multiple services. But what if you could address all of them with just one decision? A decision that results in an all-inclusive, coordinated senior care and support plan that includes healthcare, in-home services and social opportunities. A plan that could help you live as independently as possible, in your own home, for as long as possible.
That's what our InnovAge PACE program is all about. In Colorado, we want to give you the freedom you deserve by providing your complete health program through community-based and home-based services. Through early intervention, education and prevention, our goal is to enrich the lives of aging adults.
Our interdisciplinary care team can assess your individual needs and customize a comprehensive plan for you that's covered by a complete network of exceptional primary care and specialist physician services. And as your needs change, our regular re-evaluation will ensure that you continually get the care you need.
InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services), from InnovAge PACE or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for costs of any out-of-network services. All PACE centers are open Monday-Friday from 8 a.m. to 5 p.m.
Who can participate?
People who enroll in PACE must meet these requirements:
- Nursing home-eligible
- At least 55 years old
- Able to live safely at home and in the community
- Living in an InnovAge PACE service area
What services are included?
InnovAge PACE can be an excellent option for individuals with limited financial resources or declining ability to perform activities of daily living. The program provides coordinated personal and skilled care in the home, as well as in the Day/Health Centers where socialization, meals and recreation are also available. A care plan is created for each program participant by an interdisciplinary team of professionals. Based on identified needs, the team works with you and your family to create a plan of care and outlines the services you will receive. You will be treated and checked regularly, and the plan will be updated as needed.

- Adult Day/Health Center Services
- Physician on-site/medical supervision
- Nursing care
- Rehabilitative therapies & equipment
- Physical therapy
- Occupational therapy
- Speech therapy
- Meals included (light breakfast, hot lunch)
- Nutritional counseling
- Social worker advocating for health care choices
- Personal care
- Recreational activities & exercise
- Adult day program
- Education for participants & families
- Mental health counseling
- Some alternative therapies (i.e., acupuncture, chiropractic)
- Miscellaneous supplies such as oxygen, incontinence products, dietary food supplements, etc.
- PACE covers over the counter (OTC) medications if they are prescribed by your PACE physician

- If InnovAge approves your permanent stay in an assisted living facility, you will be responsible for a monthly housing payment to the assisted living facility for room and board.

How much does it cost?
There are no co-pays or out-of-pocket costs for PACE services for people with Medicaid benefits or for those with Medicare and Medicaid, unless they live in or move to supportive housing (assisted living or nursing home), in which case there is a co-pay. InnovAge PACE does offer Medicaid counseling, assistance and redeterminations to help make the program easier to understand and utilize. Many individuals can also access the PACE plan using their Long Term Care Insurance benefits, and we can help navigate through the ins-and-outs of this type of insurance. In addition, some participants choose to pay privately, so they can have access to the all-inclusive PACE program. Several options are typically available to any one person, and our Enrollment Representatives can explain them and help determine which are best under the specific circumstances.

Support for family caregivers
Respite care is available through InnovAge PACE to help caregivers who need a short break from their responsibilities. Please ask for more information.
